Employers of all sizes, whether fully insured or self-funded, can benefit from BHCG membership.  The vast majority of BHCG activities are supported through annual membership fees.  Individual or Associate memberships are available as well as membership through a sponsoring organization.

Individual Membership


Individual memberships are available to employers who have a minimum of 400 eligible participants, are self-funded and submit a joining fee. They may participate in all BHCG meetings and activities and have a voting right if they choose to offer the health plan through BHCG's Medical Administrative Partner.

Associate Membership

Associate memberships are available to employers who have less than 400 eligible participants and that join independently or through a BHCG sponsoring organization or employers that have over 400 eligible participants and join through a BHCG sponsoring organization or are not eligible for an Individual Membership. Associate members do not have a voting right but are allowed to access the health plan through BHCG’s Medical Administrative Partner and have access to information, educational materials and meetings of BHCG’s members as determined by the Executive Steering Committee. If an associate member joins through one of BHCG's sponsoring organizations, the one-time joining fee is waived, however they still pay the annual per-participant fees.

  • $5.00 per eligible subscriber** residing outside BHCG’s Covered Region****


*Joining Fee:

  • 2 - 199 = Waived
  • 200+ employees = $1,000

An employer’s joining fee is waived, regardless of the number of employees, if joining through a sponsoring organization.

**An "eligible participant" includes all active employees and pre-65 retirees who are eligible to take their company sponsored medical plan offering, regardless of carrier, even if an employee and/or pre-65 retiree chooses not to enroll. The employee and/or pre-65 retiree must reside or work within BHCG’s Covered Region (see *** below). An “eligible participant” excludes any Medicare eligible retiree regardless of the medical plan offering.

*** An “eligible subscriber” includes all active employees and pre-65 retirees enrolled in their company sponsored pharmacy plan offering through Navitus Health Solutions. An “eligible subscriber” excludes any Medicare eligible retiree.

**** BHCG’s Covered Region is defined as the twenty-two (22) eligible counties of Brown, Calumet, Dodge, Door, Fond du Lac, Green Lake, Jefferson, Kenosha, Kewaunee, Manitowoc, Milwaukee, Outagamie, Ozaukee, Racine, Rock, Sheboygan, Walworth, Washington, Waukesha, Waupaca, Waushara and Winnebago.
